As your Independent Patient Advocate and Registered Nurse (RN), we invest great amount of time learning about you, your family, and your personal routine. We assess your mental, physical and spiritual needs. We thoroughly evaluate your medical, social and family history, medications and anything that can affect your overall health and most of all, what you need and want in your care in a given day and time.
We listen to the concerns of you and your loved ones, visit the home and recommend options for improved quality of life, healthcare services, and cost containment
Ensure that medical professionals meet the client’s needs and wishes
Give clients a voice in their medical treatments and personal care and overall health
Answer questions, provide resources and walk the clients through potential treatment plan that are right just for you.
Keep patients informed about the treatments and procedures or other options
Provide guidance to case managers, social workers and other health care providers about the clients’ needs before and/or after doctors appointments, surgeries and treatments, hospitalizations, rehabilitation, hospice care or at home to increase the clients’ ability to manage their health where they are.
Request referrals to other healthcare providers when and as needed
Keep the healthcare team and family members informed of situations involving patient care such as religious, cultural or personal beliefs and the client’s goals
Protect client’s rights
Preserve human dignity and ensuring that patients have the right to make decisions about their own health
