  • Dermal patch application/removal

  • Blood sugar, blood pressure monitoring

Medication Management

  • Maintaining current list of medications and proper schedules

  • Maintaining current list of allergic reactions or side effects effects

  • Continue reassessments for potential

    electrolyte imbalances

  • Organizing and combining pill bottles

  • Reordering medications

  • Filling pill boxes. Personalized medication administration devices to optimize your health and independence.

  • Medication recommendations as needed and after careful and detailed evaluation of diagnosis, clinical presentations supported with skilled nursing assessments and laboratory/tests results

  • Requests for medication simplification when appropriate to avoid poly pharmacy and unwanted side effects

Triage of Medical Emergencies, communicating with the doctor and obtaining/following doctor’s orders. Stay with you until you are stable and safe to be at home; we are available 24/7 during critical circumstances and/or until we coordinate the care you need to stay safely at home

Management of acute of chronic illness

Pre and Post Operative Care during day surgeries or hospitalizations

Clinical Care Management and coordination

Discharge Coordination and Care from hospitals, skilled care facilities, assisting living/personal care, rehabilitation units or transfers from home to any other healthcare setting

Direct Communication with Doctors while attending medical appointments; able to make recommendations within the visits or after a thorough evaluation of the medical conditions; assessment finding; reviewing medications and results of laboratories and other tests

Keep track of old and new symptoms, problems with current treatments, or significant health or personal changes to discuss with the doctors or health care providers

Hospice and Respite Care, education and support with options to travel within or out of the country. Maintaining direct contact with your hospice team and making recommendations

Care Giver Support and Education

Non-Skilled Care: Assistance with Activities of daily living such bathing, dressing, hygiene care, transfers and/or with instrumental activities of daily living including, but not limited to  transportation, cooking, light housekeeping, medication reminders
